End of Year Reports and Evaluations

 

The following items are due to Library Services at the end of each school year:

  • End of Year Report
  • Library Program Evaluation and Score Sheet
  • Annual Summary Report for the School Library (to be given to your principal)
  • TAKS Support Plan Evaluation

 

 

Information from these reports is used to create the Library Services annual report to the School Board.

 

Library Services staff will begin running the statistical reports that are needed from the automation system after circulation has ended for the year. Librarians will be notified when this information is available and can start compiling their reports at that time. Statistics from the automation system and information from the library calendar, lesson plan book, sign-in sheets, etc. will be needed to complete the forms required.

 

**All reports must be received at Library Services before 2pm on June 6, 2008. Reports may be hand-delivered or sent via e-mail, but should NOT be placed in school mail.**

 


 

Form to complete online

 

 

End of Year Report - This online form requires statistical information about your collection, budget, materials use, library use, and collaboration.

 

DIRECTIONS: Library Services staff will run the Sirsi reports needed to obtain many of the statistics required for this form. Check here for EOY Workflows Stats. The stats gathered will show in a green shade.  Other information needed will be found in your library calendar, lesson plan book, sign-in sheets, etc.

Library Program Evaluation - This Program Evaluation is based on the Texas Standards and Guidelines for School Libraries. It is used to assign a rating of Exemplary, Recognized, Acceptable or Below Standard to the campus library program.  You will need the Library Square Footage Chart to help you with Domain IV.

 

DIRECTIONS:

1. To assign a rating, begin reading each indicator at the Acceptable level.

2. If your library program meets the Acceptable level, go on to the description of the Recognized level.

3. If your program does not meet the description for Recognized, you will assign that indicator a rating of Acceptable.

4. If it does meet the Recognized level, continue on and read the description for Exemplary.

5. If your program does not meet the Exemplary level, your rating for that indicator would be Recognized.

6. Assign a rating to each indicator listed in the Campus Score and Comments column.

IMPORTANT: If you assign a rating of EXEMPLARY or BELOW STANDARD on any indicator, you will need to add an explanation for the rating in the Score and Comments column.

 

MISD Score Sheet for Library Program Evaluation - Use this Score Sheet to calculate the overall rating for your library program.

 

 

DIRECTIONS: Take the numerical ratings from the Library Program Evaluation and enter them into the corresponding cells on the score sheet.

 

Annual Summary Report for the School Library (requires Principal's signature) - This is a one-page summary report that you will give to your principal. It takes selected information from the End of Year report and the Library Program Evaluation.

 

DIRECTIONS: Enter selected information from the End of Year Report and the Library Program Evaluation. Sign the form and ask your principal to sign also. Make three (3) copies - one for Library Services, one for your principal, and one for yourself.
